Gentlemen:
We are herewith forwarding the construction permit for the project cited above
for which plans have been reviewed and approved by this Department. Please
note the conditions and requirements under which this permit is issued.
We are retaining one set of the plans for our files, forwarding one set here-
with, and returning the remaining set to the designing engineer.
Pursuant to Section 455B.33(4) , Code of Iowa, 1977, and Rule 400 IAC 24.12(4) ,
you have the right to appeal any condition of this permit by filing with the
Executive Director of the Department of Environmental Quality a notice of appeal
and request for administrative hearing within thirty days of your receipt of
this letter and permit. If you have any questions, please do not hesitate to
contact this office directly.

On the matter of Application of:

In accordance with the provisions of Sections 455B.32, 455B.33 and 455B.45, Code
of Iowa, and Rule 400--19.2(455B) or Rule 400-20.5(455B) , or Rule 400--22.12(455B)
of the Iowa Administrative Code, the Executive Director of the Department of
Environmental Quality does hereby issue a permit for the construction of:
1642' of 8" DIP Water Main
Reed's Subdivision
Ames, Iowa
for which plans and specifications have been approved. The permit is issued subject
to the following conditions and requirements:
1. No material change in the construction of the project shall be undertaken or made
unless first authorized by the Chemicals and Water Quality Division;
2. The construction of the project shall be initiated within .one year of issuance of
this permit or this permit is no longer valid;
3. Notification of completion of construction shall be made to the Chemicals and Water
Quality Division within 30 days after completion;
4. No legal or financial responsibility arising from the construction or maintenance
of the approved works shall attach to the State of Iowa or the Department of Environ-
mental Quality due- to the issuance of this permit;
5. The issuance of this permit in no way- relieves the applicant of the responsibility
for complying with all local, state and federal laws, ordinances, regulations or
other requirements applying to this project, including the safety rules of the Iowa
Employment Safety Commission and State Bureau of Labor;
6. This permit- for the construction of the project does not constitute a permit for the
discharge of sewage, industrial waste or other wastes to the waters of the State;

7. Water mains shall be constructed in accordance with the standard specifications of the
city of Ames, a copy of which is on- file- with this Department;
8. Sufficient separation of waten.:mains and sewer lines shall be provided in accordance
with the GLUMRB "Recommended Standards for Water Works" to protect the water
distribution system from possible contamination;

with .the GLUMRB "Recommended Standards for. Sewage Works" to pxotect the water
distribution system .from. possible contamination;
1Q. " Additional maintenance shall -'be provided if found,necessary for those sewers
constructed with greater manhole spacing lengths than that recommended by "Ten
State Standards".
11. The city of. Ames shall continue engineering and financial planning for required
facilities to achieve compliance with the effluent limitations and implementation
schedule'.coratained in Iowa Operation Permit No. 5-85-03-0-01; issued June 23, 1975,
td.•the municipality": and amended thereafter. In the meantime, the city of Ames
shall .maintain. and operate 'the existing wastewater treatment - facilities in such a
manner as to obtain maximum.treatment efficiency at all times and to comply with the
.requirements of this- Department for such operation;
