NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of Ames, Iowa, that Change Order No. 5 on contract with Helfrich Brothers Boiler Works, Inc., of Lawrence, Massachusetts,for the Unit 7 Boiler Repair Project be approved and adopted as attached hereto and made a part hereof by this reference. MA 01843 2 $22,951.50 4 761,733.00 The contract is modified as follows upon execution of this Change Order. The changes included in this change order are made in order to carry out the original intent of the contract. The changes included in this change order are to be accomplished in accordance with the terms, stipulations, and conditions of the original contract as though included herein. DESCRIPTION COST Helfrich will perform some of the extra work described in the two (2) quotes Helfrich Brothers Boiler Works (HBBW) provided on August 23, 2019 labeled 'HBBW Proposal No. 19-170 CO 6 Rev.2' and 'HBBW Proposal No.19-170 C05 Rev.1.' The work to be performed is as follows (all prices are inclusive of sales tax): 1) Supply and Installation of Replacement Screen Tubes $90,275.90 • HBBW will supply twenty-six (26) new screen tubes with Inconel 625 weld overlay 0.050" thick. • The tube material is to be 2.50" OD X 0.203" MW SA210 Al. • Tubes will be straight sticks with no bends. Cut lines are to be at the tangent of the upper and lower bends of the screen tubes. • HBBW will install all 26 tubes during the August through September timeframe. 2) Supply and Installation of Replacement Seal Trough and Air Seal $244,709.00 HBBW will supply the following material for the new replacement Seal Trough • Supply in kind (1) design with upgraded material to 304 SST water seal trough assembly with W8 & C11 stiffeners and (4) drain locations. The new W8 & C11 will be provided with the seal trough and will be spliced into the exiting W8 & Cl 1. • Supply 3/16 316 SS saw tooth wear plate • Supply 10GA 309 baffle to replace Inconel drip screen • Supply scallop tube attachment • Supply air seals with gaskets and hardware • Remove and replace a 2' band of 9" thick refractory around the inside of the ash hopper approx. 22,long by 10' wide. Refractory will be replaced with equivalent materials • An inspection of the balance of the ash hopper refractory will be conducted. The inspection will identify areas of refractory that will require repair. HBBW will identify areas of concern and will go over the results with the City of Ames for approval to proceed prior to removal and repair. The refractory will be removed and replaced at identified areas and steel will be evaluated for repair if necessary. All additional refractory repairs based on inspection and will be performed on a T&M basis. • Remove existing seal trough and feed piping • Prep existing W8 and C11 members to accept new trough • Install new trough and restore feed water piping • Install scallop to new WW tubes. Scallops will be stich welded • Install new air seals 3) Supply and Installation of New Over Grate Air Plenum $53,446.50 HBBW will supply and install a new Over Grate Air Plenum per the HB-SK71819- PS-00 proposal drawing 4) Perform Front Waterwall Hanger Modifications $69,596.01 HBBW will supply and install three (3) additional Front Waterwall hanger lugs, Hanger Rod Assemblies (Hanger Rods (SA182 F11), Pins, Clevis, heavy hex nuts and washers), and modify the existing back to back channels to take into account the additional weight of the overlay. Pricing includes engineering design of the placement of the hanger lugs and reinforcement of the existing channels, installation of the new hanger lug plates to the front waterwall tubes in the penthouse and installation of the additional reinforcement plates on the existing channels the hanger rods hang from. HBBW will issue drawings that are stamped by a Professional Engineer in the state of Iowa.
