CONSENT TO PLATTING
Dedication and Conveyance to the Public

COUNTY OF STORY
COMES NOW the undersigned and hereby states that he is the owner of the
following described property located in the County of Story, Iowa:
(See Exhibit "A" attached hereto and by this reference incorporated
herein)
which is to become known as Third Addition to Country Estates, Story County,
Iowa.
It is hereby acknowledged that said platting and subdivision as it
appears on the plat was prepared with the free consent and in accordance with -
the desire of the undersigned.
The undersigned hereby dedicates to the public all lands within the plat
that are designated for streets, alleys, parks, open areas, school property,
or other public use, if the dedication is approved by the governing -body, and
specifically described as Outlot H (North Fork Road) and Outlot G (Right-of-
way along Dayton Road) , provided however, it is understood that Story County
will not maintain said improvements.
Pursuant to Iowa Code §354.11, the above dedication is, upon approval of
the governing body, equivalent to a deed in fee simple from the undersigned
and the undersigned hereby convey to the public any land within the 'plat that
is dedicated for the above-described public uses, with waiver of all rights of
dower, homestead and distributive share.
Upon approval of this dedication of land for street purposes, an
easement is established and the undersigned hereby grants an easement for
public access whether or not a deed is recorded, or the improvement of the
street is complete.
The recording of the subdivision plat shall and the undersigned does
hereby dedicate to the public any utility, sewer, drainage, access, walkway,
or other public easement shown on the plat.

APPROVAL AND ACCEPTANCE BY BOARD OF SUPERVISORS
The foregoing Consent to Platting and Dedication and Conveyance to the
Public of all lands within the plat that are designated for streets, alleys,
parks, open areas, school property, or other public uses, easement for public
access to streets, as well as utility, sewer, drainage, access, walkway or
other public easement shown on the plat, shall not be binding until it has
received the final approval and acceptance by the Story County Board of
Supervisors by Resolution, which approval and acceptance shall be noted on
this Dedication and Conveyance by the Chairperson for the Board.
