The worksheets list the offices that will be filled at the November 3, 2009 City Election. The information provided on each worksheet shows the office on the ballot, the number of persons to be elected, and the number of signatures that are required for a candidate to be nominated. This information has been compiled from the election records in the Auditor's Office. Please find your city worksheet, examine the list carefully and notify me immediately if you find any errors. You may print off your city worksheet, sign it and fax it to me, or simply reply to the email indicating if your worksheet is correct or if there are updates that need to be made. Please reply or fax your worksheet by Monday, August 17. Vacancies occurring on or before Saturday, September 12, 2009 must be filled at the November election, unless the vacancy was filled at an earlier, special election. If you have a vacancy that was filled by appointment, that position must be on the ballot. Please notify us immediately if you experience a vacancy on or before this date. All candidates' nomination papers and affidavits, including papers for the unexpired term, are to be submitted to you on Thursday, September 17 by 5:00 p.m. These are to be delivered to the Auditor's Office on Friday, September 18 by 5:00 p.m. This includes full text for any public measures on your ballot. • We have a special request . We would greatly appreciate having your paperwork turned into our office as early as possible on Friday, September 18. We have a unique situation this year pertaining to a software change. Early delivery of the ballot information will allow us additional time to create the ballot in order to send it to the printer in a timely manner. If delivery of ballot information is not possible until the afternoon of Friday, September 18, a faxed copy of each Affidavit of Candidacy and ballot language sometime during the morning would be greatly appreciated. Our office fax number is 515-382-7221. Our hours are 8:00 AM — 5:00 PM. Many thanks for your consideration of this special request. As the candidates pick up their papers, please tell them to type or print the information at the top of the Affidavit of Candidacy for City Elections and the Nomination Petition for City Office Candidates. Names need to be exactly the way the candidates want them to appear on the ballot. • The following link to the Secretary of State has copies of the candidate forms available for printing. http://www.sos.state.ia.us/elections/electioninfo/cityelections.htmI • Campaign Finance and Disclosure brochures are now available at www.state.ia.us/ethics. • You may find additional election and voter registration information on the Story County Auditor web page at www.storycounty.com under the Auditor and Elections page. When a candidate returns papers to your office, be sure each form is filled out correctly and completely. Please see the City Clerks Guide for procedures on how to receive in nomination papers. When candidates return their papers, please give them a copy of the attached Candidate Profile Letter. This document is optional for the candidate but provides a helpful source of information for voters about the city candidates. Your assistance is essential in planning successful city elections. The Story County Auditor's webpage is a widely-used source. We have a Candidate Profile section to provide citizens with standardized information about candidates. Submitting a profile is optional. All candidates appear on the Candidate List which is available in my office as well as the webpage. If you choose not to submit a profile, your name,address,and office sought will still be available on this document. The candidate profiles are managed via a web-based application created by Story County's Information Technology staff. To submit a candidate profile,you must provide a valid email address on your candidacy papers. We will initialize the profiles once the filing period has ended. You will receive an email containing web links allowing you to complete the candidate profile. Security is a high priority.The web link will only work once. If you need to edit your profile,please contact my office. Submitted profiles are reviewed by my staff before they are posted to the website. Candidate profiles are then posted to a secured site. The purpose of the candidate profile is to provide prospective voters with general information about the candidates; it is not intended to solicit votes,raise funds,or advocate in any way. Each profile is allotted identical space for consistent content. This will distinguish the vacancy from other seats on the ballot that have the same title, but have terms that end at a later time. For example: Council Member, To Fill Vacancy. Each candidate must file a nomination petition with no fewer than the minimum number of signatures. Each candidate must file an affidavit of candidacy with the nomination petition.
